This is a customized program for 6 months that includes 4 Classical Chinese Medicine session to start you off.

II. 48 HOUR CANCELLATION POLICY
This 48-hour cancellation policy is a courtesy for others who might be in need of care or the use of treatment rooms. You will be charged a fee of 40% on the service booked if we failed to receive your cancellation. Due to unforeseen government policies, your appointment might be moved. If you opted out email from electronic record Unified Practice application, you might receive delayed notice.
III. SERVICE POLICY
Our service policy indicates that if member should cancel or not show up for three appointments within a three-month period, you will be referred out to other physician for care of your condition/s and will not hold Heavenly Stems & Earthly Branches for negligence or refusal of care.
IV. SAFETY POLICY
We reserved the rights to stop treatments. Bodywork and cosmetology services are non-refundable. When there is a health condition in our medical opinion that requires further assessments, your treatment may be delayed or reschedule. Your medical care may also be provided by herbal treatments and or telehealth. At the same time, we do the utmost to safeguard your medical history and treat them confidentially.
V. MEMBERSHIP POLICY
Within 30 days, we will refund you of your remaining unused service value. After 30 days, the refund amount will be your remaining balance value minus 15%. After 40 days, the refund amount will be your remaining balance value minus 40%. After 120 days, no refund. Prepaid membership is transferrable between family members. It holds no cash values. All membership expires after 365 days of initial purchase. For record keeping, you must initial the service amount after you have received treatment/s. Corporate membership is available to companies with less than 50 employees.
VI. PAYMENT
Acceptable forms of payments are US cash, for charges over $100 credit or debit cards are accepted, HSA or FSA only for acupuncture services. Zelle or minted 2022 Silver American Eagle - 1 Troy Ounce, .999 Pure or 1/10 Oz American Gold Eagle Coin, Old Style. Personal checks are not accepted. Superbills or receipts are emailed to you.
